Proof of Vaccination: COVID-19 Digital Certificate

In most cases, a COVID-19 digital vaccination certificate will be automatically generated after an individual has their second dose.

Information for those that do not have a Medicare card and are needing to obtain proof of vaccination can be found here:

If someone has had a 1st and/or 2nd dose at a state-run vaccination centre and their immunisation history appears to be missing or incorrect in MyGov, they can complete the form on this website: https://www.coronavirus.vic.gov.au/missing-immunisation-history
